We are merging our medical business with two other Wellington practices.
From Monday 22nd November as a medical patient, you will be
automatically enrolled at the new practice at 21 Mahora Street Kilbirne
(Ph 04 387-9758). The clinic will have both male and female doctors available,
Alvin and Adrienne Mitikulena
Your medical records will be automatically transfered to the new practice. If
you wish to enroll with another medical centre of your choice, please ask the
receptionist for the phone number for accessing information on clinics taking
new patients.
We are negotiating to have clinics available at our Lukes Lane location in
the new year.
Membership, advocacy, complementary health and dental services remain unaffected by
merger of the medical service.
Data Protection / Confidentiality
Your personal information is as important to us as it is to you. We collect
and store data in both paper and electronic forms.
Your medical records are held in the strictest confidence.
Privacy act - In accordance with the Privacy Act 1993, your personal
information will not be passed on to any other source without your permission.
You have the right to review and correct any information held by us. Please let
reception or the nurse know that you wish to do so and we can have the correct
form signed by you and a time to do so will be given, with the supervision of
the doctor or nurse.
Violence & Aggression
We operate zero-tolerance policy toward any form of aggression or violence
towards any member of the practice team, or patient/ visitor on our premises.
This includes all forms of physical, verbal or threatening behaviour.
Anyone threatening or abusing any member of the team may be removed from the
practice list, or other restrictions placed upon the services we provide to
them.
Complaints Process
It is important to us that members are able to make complaints and
suggestions so we can provide the best possible service.
Any complaint can be made by speaking to a staff member, preferably the
Health Services Coordinator, or to the People's Centre Management Committee
through its chair, if necessary by mail ( Wellington People's Centre,
PO Box 9491, Te Aro, Wellington 6141 ).
All complaints will be taken seriously. They will be acknowledged within
three working days and provided with a full response within two working weeks
following investigation of complaint details.
